I've added yahoo to my Bookmarks toolbar and it's still there. When I try to bookmark another page, upon closing the browser, the added bookmark in the toolbar will dissapear.
Are you using the K-Meleon loader? You could try disabling the loader before you make any changes or additions to the toolbars, as it always loads the previous configuration.
Some questions:
Which OS ?
Where is located the profile, and obviously does a profile.ini exists in K-Meleon folder ?
Have-you rights to write in the folder where is located the profile?

Not sure if that sounds a bit misleading... If the loader is running, it makes that the browser isn't closed completely, as long as the loader isn't closed too. That's to keep some core portions of KM in memory, to get it to start faster next time.
Of course that means that changes which require a browser restart, will take effect only after a 'full' browser restart, that means after restarting also the loader (if a user has enabled that function). But those changes are not lost or forgotten, and it doesn't matter if the loader is closed before or after making any changes. Either way they will show at the next full restart.

That the bookmarks bar does not work is probably a bug. Else I 'd moved the other one here.
The rest is IMHO the FAQ: No Writing Rights.
Symptoms: Either or all - altered settings, Bookmarks or Hotlist do not stick/write.
So the new settings are gone after the next browser restart. Bookmarks do not write...
Occurrence: All Windows NT based systems - especially Vista and Win7 where ppl do not habitually run their systems as admins.
Especially common when ppl use the "portable" 7z of 1.6.beta2 that has a profile.ini by default.
Solution: delete profile.ini or grant writing privileges to profiles folder.
@alain: 9x users are rare nowadays, have their system for long and know what they are doing.
Maybe alter the installer so that it says something like:
Portable Setup
Needs permanent writing privileges in the install folder.
Else: For setup on removable media with writing privileges (& for Win9x with kernelx).
