File Handling Settings

I am used to the setting that clicking on a link with a PDF-file behind I get a small "how to proceed"-window to ask me whether to
a) open the pdf-file with (external) PDF-Reader,
b) save the file to disk, or
c) do neither nor and cancel the operation.
I DO NOT want to open it inside the browser with a plugin (or else).

Now I changed the setting from "use plugin" to "open with default program". As there is no warning before opening it, that's not the solution. So I wanted to set it again, to "save to file"; but I can't: it's gone and no longer exists there ...

And there is no button or possibility to add another file type ...

Now two questions:

a) how to add an additional behaviour for another (not yet mentioned) file-type?

b) how to set any kind of file-type to "ask whether to open OR save to file with the option to cancel the operation"?

1) type about:preferences in URLbar and go there - Mozilla settings page will open
2) select Applications in the left menu column
3) find PDF filetype in table and set it to "Always ask'
