My back navigation button never appears active anymore (after page has loaded). I can still right click and click back, or Alt+left arrow.

Sorry, beta tests for this bug ended more than ten years ago. It was added to test beta testers. Close tab and continue in a new tab.
I dimly remember some users here disabled that shortcut intentionally, when there was a bug in an older KM-version, related to typing in textareas, because hitting the backbutton flipped back the whole page...
Check if you have it redefined too. Probably in accel.cfg (either in profile or in defaults/settings folder). Or it could be in a macro, but would be unusual.
Or you can test what happens in textareas when you add this in accel.cfg in profile:
VK_BACK = ID_NAV_BACK
