Building a toolbar from scratch

How do I build a toolbar from scratch ?
does the bmp need to be a certain size, How do I sepwrtae the individula buttons etc etc?
A step by step guide would be useful (or a link to one )

Most of the details are in the toolbars.cfg file. If someone wants to write up a doc, we can add it to the site.
