If you open up KM75.1 Portable and lock the toolbars (or don't), uncheck Preferences>K-Meleon Plugins>Netscape Bookmarks>â˜Enable support for Netscape Bookmarks, and proceed to enable Preferences>K-Meleon Plugins>IE Favorites>
☑Enable support for Internet Explorer Favorites, your Previous tab, Next tab, New tab, and Close tab buttons will be moved up one toolbar.
This happens to me in random situations, too.

KM 75.1 Portable starts with tabs manager in the wrong place but fixes with IE Favorites enabled.
