nothing more to add...don't see a pattern in it at all. play with "back" and "forward". maybe it has something to do with "stop", too...
Sometimes you go "Back" and in the Title and the URL field of K-meleon you still got the data from the old page. Can't see a pattern in it too...
Did you select Tools|Preferences|Cache|Cache Comparaison to be on always?
If not this may solve your problem, at least it solved mine.
