1. It depends in the context. From macros they must be called "&macroname;" but from menus.cfg or accel.cfg (and setmenu, setaccel macro functions) there is no need. 2. The idea is that it became broken after new Gecko engines implemented in 7x.y versions. You can manage them better through about:preferences url. Most of the times might works better if you just remove the file type mime definition than setting a preference (ask, download).
